1. Do I need a referral from my doctor?

No referral needed! North Dakota is a state that allows you to go directly to a physical therapist without getting a physician’s referral first.

Direct access to physical therapy is your opportunity to be evaluated and treated by a licensed physical therapist without first seeing your physician for a prescription. If payment is approved by your insurance company, direct access is your opportunity to save time and money, thereby expediting your treatment, relief, and recovery.   Research supports that patients who seek physical therapy via direct access for a musculoskeletal condition tend to have better functional outcomes with fewer treatments and lower overall health expenditure.

2. What are my payment options?

Optimum Therapies accepts most insurances. Insurance coverage may vary depending on group plan chosen by employer. Always verify group participation when verifying your benefits. We accept cash, check or credit card. Co-payment/co-insurance/deductibles are required at time of service. 

3. What will happen at my first visit?

Plan on 45 minutes to 1 hour for your first appointment. The appointment will be an evaluation and will consist of a one – on - one assessment of your condition/impairment.

After the assessment your physical therapist will treat and educate you on the condition/impairment and send you home with an exercise program. You and your physical therapist will decide on a plan of care that includes frequency of therapy, duration of therapy and treatment options available to your specific case.

You will be asked to register online, If you are not able to complete this form, please check in 15 to 20 minutes early.

Part of your therapy evaluation includes a functional questionnaire. This questionnaire helps your therapist maximize your improvement during therapy. It is called FOTO  – Focus on Therapeutic Outcomes – and this information helps our team focus on getting the best results for you. You will also be asked to update this FOTO questionnaire throughout your treatment to see how much you are improving.

4. What should I wear to my first visit?

Wear loose comfortable, clothing such as workout clothing with tennis shoes. We do have shorts and t-shirts available for use during your appointment.

5. How will my Primary Doctor follow my progress?

At Optimum Therapies we are a true believer of collaborative care. Your physical therapist will communicate with your primary doctor and specialist from start to finish.
